Three authorities have initiated a joint investigation into the events leading up to the LynnMall stabbings.
The Independent Police Conduct Authority, the Office of the Inspectorate at the Department of Corrections, and the Inspector-General of Intelligence and Security will investigate how Ahamed Samsudeen was handled in detention – and in the seven weeks preceding up to the attack.
The investigation will look into whether police, Corrections, and the NZSIS did enough to assess and respond to the risk posed by Samsudeen.
A separate review is already underway into the decision to shoot Samsudeen.
The coordinated review is due for completion next March.
